Sign Up Form – Current Students (2025-2026 School Year)

Welcome back! We’re thrilled to have you and your child with us for lessons starting in September 2025! Please complete the following form to register for the 2025-2026 school year, even if you have already discussed with your teacher.

We will make every effort to accommodate your preferred schedule. If your availability doesn’t align with your teacher’s, we will reach out to discuss alternatives.

Release of Liability: In consideration of services, I, for myself, for my child, do hereby release Willan Academy of Music, its principals, directors, officers, agents, employees, and the individual members of the consultation team from all liability with respect to my child and I waive any claim for damages arising from any cause whatsoever.

Photo/Recording Consent: By enrolling in Willan Academy of Music, the student or parent/guardian (for students under 18) authorizes the making and use of any photos, videos or sound recordings during his/her participation in Willan Academy of Music concerts/events for any purpose Willan Academy of Music may make or authorize without compensation to the student or parent/guardian.
