Consent Form for Videotaping Student Recital

I hereby give my permission for my child to be videotaped during the student recital on May 21, 2023 at Dimenna Center, 450 W 37th St, New York, NY 10018.

I understand that the footage will be shared with the students and their families, and will also be used for promotional purposes by the music school on their website, social media, and other platforms. I also understand that the music school will use the footage in an anonymous manner, and will never disclose the names of the children in the footage.

I grant the music school and its representatives the right to use, reproduce, distribute, and display the footage of my child in any form and for any purpose, without any compensation or obligation to me or my child.

I release the music school and its representatives from any liability or claims arising from the use of the footage of my child.

I have read and understood this consent form and I sign it voluntarily.
